What is the difference between standard, gold and extreme rides.

I am ok with standard rides, I think.

If I understand correctly Gold rides are standard rides done in a shorter time frame.

What elevates a ride to extreme, not gold? Is there a set rule or is each one judged on its own merits?

EDIT what makes a ride insanity level?

A Quick Example for a local ride Gordon is the European End to End .. Standard, Gold and Extreme(Insanity) levels.
